What is an Enhanced Primary Care (EPC) Program?
An Enhanced Primary Care (EPC) Program are a means of General Practitioners (GPs) to manage health care (including multidisciplinary care from a GP and Allied Health Services) to patients with chronic or terminal medical conditions (a medical condition that has been or will be present for a minimum of 6 months). This makes patients eligible for Medicare rebates for certain Allied Health Services through the GPs referral.
How do I get an EPC Program referral?
The Enhanced Primary Care (EPC) program will be provided by your GP if you are deemed eligible for this service following a clinical judgement from the GP after considering a patient’s medical condition and healthcare needs. This program allows for a maximum of 5 Allied Health Service rebates per year.
What do I do once I receive the referral?
After obtaining an EPC Program referral from your GP, Modern Physiotherapy will be ask to be provided with the according documentation and therefore commence Physiotherapy services.
How much will I need to pay?
Once accepted, you will have the option of:
- 5x 30-minute consultations with our Registered Physiotherapist: bulk billed. (Please note, Radial Shockwave therapy treatment will cost $30 extra per treatment and our Specialist Physiotherapist does not provide treatment under EPC care plan .)
